Students,
Please keep in mind that all program changes will be honored for the following reasons ONLY:
1. You are currently sitting in a class that you have already passed.
2. You are missing a major subject, which is Math, Science, English or History.
You must continue to follow the program you have until you receive approval for a new program.
ALL CHANGES ARE DUE FEBRUARY 1ST BY THE END OF ACADEMIC ADVISORY AT 2:40PM.
Your request will be processed the week of February 4th. You will be notified by your guidance counselor. Please note that schedules and add/drop depend on class availability, your academic requirements and transcripts, and class size.
Note: You and a parent and/or guardian must review your credits and requirements before signing this Program Change Request Form. Final approval will require parent signature.
